Every tutor has had this conversation a hundred times. A student misses six questions on a practice test. We review them together, and for four of the six the student says some version of "oh, I KNEW that one." Dropped a negative sign. Distributed wrong. Solved for x when the question asked for 2x.
Students treat these as personality flaws. "I'm just careless." They are not personality flaws. They are process flaws, and process can be changed.
The Biggest Fix: Stop Doing Work Desmos Can Do
Here is the thing I push on students more than anything else, because after a decade of tutoring it is the single highest-value habit I know: you are not using Desmos enough. Not as a fancy graphing trick for special occasions. As your default.
Any time you have an equation, you can just put it in Desmos and it will solve it for you. Type the equation, see the intersection points, read off the answer. That is it. Every algebra step you did not do by hand is a sign error you cannot make, a distribution you cannot botch, an arithmetic slip that cannot happen. You totally eliminate the human-error aspect of test taking.
In our video explanations I show how to do almost everything in Desmos, and this is exactly why. I want the human-error surface of the test as small as possible. Some students resist because they can do the algebra faster by hand, and sometimes that is even true. But here is the trade they are not seeing: even when Desmos is slightly slower, if it makes it obvious whether your answer is right or not, it is usually still worth doing it in Desmos. Ten seconds of extra typing that converts "I think it's B" into "it is definitely B" is the cheapest insurance on the test.

For the Errors Desmos Cannot Catch
The remaining careless misses are almost all reading, not math. Solving for the wrong quantity, missing a "NOT," using the wrong units. The fix there is a five-second habit: before answering, reread the actual question sentence and confirm your number answers it. Not the whole problem. Just the question.
And track your misses honestly. When you review a test, sort every miss: concept gap, misread, or mechanical slip. If mechanical slips are a big slice, that is not a "study more" problem, and no amount of content review will fix it. It is a "change the process" problem, which is better news, because process changes stick faster than new content.
